Dr. Allan Mishra has been an orthopedic surgeon for over twenty years. He’s on faculty at Stanford, he’s spent decades researching regenerative medicine, and he hosts his own podcast on vitality and performance. By most measures, there isn’t much left in this field that should catch him off guard.

What surprised him?

The fact that fat is not inert storage, but living, reparative tissue, richer in stem and signaling cells than bone marrow, already earning a place in plastic surgery and dermatology, and showing early promise in conditions like knee arthritis.

I’d never met Allan before this Summit. I’d seen his Vitality Substack, and I knew he’d been on the faculty at the TOBI meetings. It’s a quiet joy to watch someone like him arrive, independently, at the same regard for this tissue, this science, this way of gathering people around it, that got us building ARTS in the first place.

There’s another thread in his work worth noticing. Allan’s whole practice outside the OR — the podcast, the book, the courses at Stanford — is built around vitality and sustainable performance for people carrying a lot. It’s mostly aimed at his patients and his audience. But it points at something we’ve been leaning toward for ARTS too: caring for the caregiver. The clinicians in our world spend their careers helping other people’s tissue regenerate and rarely turn that same rigor toward their own restoration. It’s not just self-care for its own sake, either. These adaptations are often invisible. Surgeons are exceptional at compensating in real time. But every micro-adaptation is a small tax on attention, a small variance in technique, a small inconsistency that can ripple into outcomes.

The most interesting innovation in regenerative tools right now isn’t about doing dramatically new things. It’s about removing the friction that surgeons have quietly tolerated for years.

A Case Study in Quiet Refinement

The premise is simple. Traditional Luer-lock connections — including earlier Tulip designs — perform well in controlled conditions. It’s the kind of small failure point experienced surgeons have learned to work around without expecting it to be solved.

The AlphaLuer design rethinks that interface. Rather than relying solely on rotational friction, it introduces unidirectional resistance within the Luer-lock connection. Once engaged, it resists reverse motion — even under the conditions that loosen traditional locks.

• Greater confidence under lateral stress

The technique itself doesn’t change. The workflow doesn’t change. What changes is the elimination of a small failure point that physicians had previously accommodated.

This is what mature regenerative practice looks like. Not a reinvention of the procedure — a refinement of every interface around it.

Why This Matters for the Field

Regenerative medicine sits at a particular moment. The science is advancing faster than the standardization. New biologics, new processing methods, and new clinical applications appear regularly. The temptation is to chase the novel.

But the field will be judged, ultimately, on consistency. On reproducibility. On whether a procedure performed in one clinic resembles the procedure performed in another. On whether outcomes are predictable enough to stand behind.

Consistency comes from a thousand small refinements — in technique, in protocol, in tools — that compound over time.

The instruments matter. The connections matter. The friction points matter.

I came across a paper recently that stopped me. Not because of what it proved, but because of what it suggested.

Two women, both over 45. Years of infertility. Multiple failed IVF cycles. They underwent a procedure involving mechanically processed adipose tissue and PRP injected into the ovaries.

Months later:

One conceived naturally. The other conceived through IVF and delivered a healthy baby.

If you work in the medical field like me, especially in anything related to regeneration, you already know what to do with this.

You say:

• It’s a case report

• There’s no control group

• It could be coincidence

• It requires further study

And all of that is true. But it’s not the part that interested me. What interested me was this:

What changed?

Not the patient’s age. Not their genetics. Not the diagnosis. What changed was the tissue environment.

In a recent email conversation, Dr. John Cole said something that felt like it named this directly. He described the distinction between:

• isolating components

• preserving the full regenerative milieu

Not just cells, but:

• cytokines

• extracellular vesicles

• extracellular matrix

• and even the mechanical context

And how different approaches influence not just immediate outcomes, but long-term tissue behavior.

Dr. Cole’s language stuck with me, because it shifts the focus away from: “What are we putting in?” And toward: “What are we preserving, or disrupting, in the process?”

The authors of the paper describe mechanisms like:

• angiogenesis

• immune modulation

• paracrine signaling

In other words, they didn’t replace anything. They altered the conditions. And something responded.

We often talk about degeneration as if something is permanently gone, lost or depleted. We talk about it as if what was there is no longer available.

That may be true in many cases. But what if, in at least some cases, that’s not entirely accurate? What if degeneration is not the absence of function, but the presence of a different pattern of tissue response?

A pattern shaped by:

• time

• repeated injury

• incomplete restoration

• accumulated scar

• lack of adequate blood supply

So the natural next question is: What if that pattern isn’t fixed?

In ongoing conversations with clinicians and researchers, we keep returning to the same idea: Tissue doesn’t just receive treatment. It responds. And that response has direction: toward stability, toward fibrosis, toward something that looks more like restoration.

So when I read this paper, I didn’t see proof of a new therapy. I saw a question becoming harder to ignore: Under what conditions does tissue respond differently than we expect?

What’s interesting here isn’t that something “worked.” It’s that something that seemed unlikely…became possible. Not immediately. But over time.

That detail matters.

Because it suggests we’re not looking at an event. We’re looking at a shift in trajectory. And once you begin to see that, it becomes harder to evaluate outcomes in the same way.

Not: Did it work?

But: What direction did the tissue begin to move?

Which brings us back to John’s point. If we isolate components, we may gain precision. But we may also lose something harder to measure: the coherence of the system itself.

And if we preserve the full milieu, we may not fully control it. But we may be working more closely with how tissue actually behaves. Especially over time.

That’s not a conclusion. It’s a tension. And maybe an important one. Because across specialties – whether we’re working with joints, skin, fat, or reproductive tissue – we’re facing the same underlying question: What is the tissue doing over time?

And, can that direction be influenced?

I don’t think this paper answers that. But I do think it sharpens the question.

The authors raised an important concern:

Are we using the word “regeneration” too loosely?

They made a clear distinction:

• Regeneration restores tissue to its original structure and function—without scar

• Repair stabilizes tissue through fibrosis and remodeling

Both are natural. Both have clinical value—but they are not interchangeable.

What struck me wasn’t just the letter itself. It was how closely it reflects what I’ve been hearing in my conversations. Across different specialties, clinicians are describing similar patterns:

• Facial tissue that no longer behaves predictably after repeated filler use
• Chronic tendon injuries that improve symptomatically, but not structurally
• Post-radiation tissue that stabilizes—but does not restore function
• Joint pain treated successfully in the short term, while underlying degeneration continues

The tissue is still there. But it behaves differently.

As an example, in orthopedics, corticosteroid injections are widely used to reduce pain. They work. But we also know—from both literature and clinical observation—that repeated exposure can contribute to cartilage degeneration over time.

So the intervention is effective, but not regenerative. It is influencing the tissue toward a different biological response.

Recently, much of medicine—and especially aesthetics—has been organized around “regeneration” delivered under the name of:

• An injectable product

• An energy device

• An implantable product

But a different question is now being asked:

What biological response are we actually asking the tissue to produce?

Because not everything that stimulates tissue is regenerative. And not every visible improvement reflects restoration of native tissue.

If 2026 is, as many are suggesting, the year regenerative tissue therapies move into the mainstream, then this distinction becomes more than academic. It becomes practical.

Because as adoption increases:

• patient expectations increase
• clinical outcomes are compared more closely
• long-term tissue behavior becomes more visible

This isn’t about labeling treatments as good or bad. Scar formation is not failure.
Fibrotic stabilization is not wrong. Remodeling can be clinically valuable.

But they are different outcomes. And understanding those differences allows for:

Many of my physician friends tell me patients are not just asking for cosmetic results anymore. They’re asking about restoration.

The Role ARTS Has Been Building

When I founded the San Diego Academy of Regenerative Therapies & Science (ARTS) nearly ten years ago, regenerative therapies were still viewed by many as experimental.

There were early pioneers.
There were skeptics.
And there were clinicians quietly exploring how biology could support tissue restoration beyond traditional approaches.

Over the past decade, I’ve focused ARTS on one goal:

helping physicians integrate regenerative therapies responsibly into real clinical practice.

A newly published clinical study offers another way of looking at this problem—not by trying to silence the nerve, but by restoring the environment around it.

The Study at a Glance

Researchers followed 19 patients suffering from secondary occipital neuralgia—a form of chronic head and neck pain that develops after trauma, surgery, or scarring in the scalp. These patients had lived with pain for years and had not found lasting relief from medications or injections.

Instead of further nerve destruction or ablation, the clinicians used a minimally invasive autologous fat transfer procedure. In simple terms, this means:

• A small amount of the patient’s own fat was gently harvested

• That fat was carefully placed beneath scarred areas around the affected nerves

• The goal was not cosmetic volume, but tissue restoration and nerve freedom

After one year:

• 18 of 19 patients experienced more than 50% pain reduction

• Over half became pain-free

• Pain episodes dropped by 85%

• Medication use dropped by 74%

• No complications were reported

These results were published in Pain Practice in 2026 by Pietramaggiori and colleagues.

One of the physicians involved in this work once said something that has stayed with me: “Nerves love fat.”

At first, it sounds almost too simple. But clinically, it captures something precise. Healthy fat tissue provides glide, insulation, and biological signaling — all of which matter deeply to nerve function. When scar tissue replaces that environment, blood supply is reduced and nerves can become tethered, compressed, or irritated. Restoring a healthy tissue layer where blood can flow isn’t cosmetic; it’s functional. In that sense, the phrase isn’t poetic so much as anatomical.

What’s Different About This Approach?

Most conventional treatments for nerve pain aim to block, burn, or numb the nerve.

This approach does something else entirely.

It restores the soft tissue layer that allows nerves to glide, reduces adhesions from scar tissue, and may support healing through the biological activity naturally present in fat—especially the cells involved in tissue repair and regeneration.

Rather than forcing change, the procedure appears to remove obstacles that prevent the body from doing what it already knows how to do.

Why This Matters

• It reframes treatment: Pain relief doesn’t always require more suppression or destruction.

• It supports regeneration, not replacement: Using the patient’s own tissue reduces risk and respects biology.

• It opens new options for patients who have exhausted standard therapies.

• It reflects a broader shift in medicine—toward working with the body rather than against it.

This study doesn’t claim to be the final answer. The authors themselves emphasize the need for further research. But it does offer something rare: a credible, clinically grounded example of regeneration changing real-world outcomes.

What I’m noticing now is how much becomes available when that urgency falls away.

Regeneration is not new. It has never been lost. It is simply quieted when we overmanage, override, or insist on improvement rather than the more subtle, yet effective, act of participation. When we try to make regeneration happen, we often interrupt the very intelligence we’re hoping to engage.

This space is not an attempt to move anyone toward anything. It isn’t here to persuade, convert, optimize or monetize. It’s a place to notice what regeneration looks like when we stop interfering with it—when we allow the body, the tissue, the system to speak in its own timing.

Across medicine, I sense a subtle change underway. A veering back—not toward novelty, but toward trust. Toward listening. Toward remembering that biology has always known how to repair, adapt, and respond when the conditions are right.

The timing feels precise. Not because the tools are new, but because our posture is changing.
